KUPUNA   CARE
Home and Community-based Long Term Care Program


 

Kupuna Care is a state-sponsored program developed by the Executive Office on Aging in partnership with the County area agencies on aging to address the growing numbers of elders with long-term care needs. On Oahu, the program is administered by the Elderly Affairs Division (EAD), Department of Community Services, Oahu's area agency on aging.


REFERRALS
Any individual may make referrals by calling the EAD Senior Helpline at
(808) 768-7700 or the contracted service provider. Refer clients with multiple needs to a Case Management service provider (see below).

 

CORE SERVICES
Eight core services - adult day care, assisted transportation, attendant care, case management, chore, home delivered meals, housekeeper/homemaker, and personal care are provided by contractors of the Elderly Affairs Division. They are intended to help older adults who cannot live at home without assistance from family and/or formal services.

 

ELIGIBILITY

ADL - Activities of Daily Living (eating, dressing, bathing, toileting, transferring, mobility)

IADL - Instrumental Activities of Daily Living (preparing meals, shopping, managing money, using the telephone, doing housework, using public transportation, taking medications)

 

COST

 

SERVICES AND CONTRACTED SERVICE PROVIDERS

Each of the eight services are provided island wide, except where noted. Clients receiving a single service will be assessed by the service provider. Clients receiving more than one service will be assessed by the Case Management service.

Adult Day Care - support for functionally impaired adults in a supervised daytime setting. Intent of Kupuna Care service is to provide short-term intermittent respite for caregivers.

Licensed Adult Day Care Centers on Oahu, starts Fall 1999.


Assisted Transportation - Door-to-door service for those with physical or cognitive difficulties preventing use of regular transportation.


Chore and Homemaker/Housekeeper - assistance with housework.
